VX FT Expansion Slot 4 Failure

There has been a small number of reported failures of expansion slot 4 on the backplane of VX FT systems. This failure has been identified as a batch of faulty PALs, part number 12157231 {TBPA006} in location IC6 on the backplane.

IC6 is a pluggable device and replacements may be organised through normal warranty/ maintenance routes.

This ONLY applies if expansion cards are not recognised when installed in slot 4.
